PHYSICAL HEALTH CARE
The University of Pennsylvania Student Health Services provides primary medical care for all Curtis students. The fee for the 2007-08 year is $800, which includes primary care visits. Student Health Services is located on the lower level of the Penn Tower Hotel at 34th Street and Civic Center Boulevard. To make an appointment, call 215-662-2853. The SEPTA #42 bus, which stops at 18th and Walnut Streets, can take you to the Penn Tower Hotel. Student Health Services also provides a shuttle, which stops at 20th and Locust Streets in the evenings, to take you to its offices. In addition the SEPTA #21 bus, which also stops at 18th and Walnut, can take you to 34th and Walnut, two blocks from the Penn Tower Hotel. The student services office will reimburse students for any cab fares to and from UPSHS upon presentation of a dated receipt.

A regular exercise program is recommended for all students. Physical fitness particularly benefits musicians who participate in the prolonged physical activities associated with rehearsing and performing. Curtis encourages you to enhance both your general health and performance-related health by participating in a program of routine exercise. Start to develop lifelong habits of exercise now to help reduce injury and improve your professional performance. For details about Curtis's gym reimbursement program, see the director of student services.

REQUIRED IMMUNIZATIONS
You are required to provide records of immunizations, including a recent TB test. If you catch measles, mumps, chicken pox, hepatitis, or any other contagious disease, or if you realize that you have been exposed to one, please immediately notify the director of student services and the registrar so that the school can make sure you are being cared for properly and precautions can be taken to protect your fellow students.

MENTAL HEALTH

Those initial consultations and counseling are at no added cost, being included as part of the services covered by the Health Services Fee. You may contact Dr. Ellen Wright directly at 215-732-3720. Her office is nearby, just off Rittenhouse Square. All conversations with her are strictly confidential.

STUDENT HEALTH INSURANCE REQUIREMENT
Curtis requires all students to have adequate health care insurance for twelve months each year and offers a comprehensive student health insurance policy. The premium cost is $2,279 for the 2007-08 school year (twelve months of coverage, September 1, 2007, through August 31, 2008). All Curtis students are automatically charged for this plan. However students may request exemption from enrollment in the plan and its cost if they feel that a parental or private health insurance plan provides adequate protection while they are attending Curtis.

To request exemption you must: (1) submit a copy of your insurance company's Summary Plan Description (SPD) or Schedule of Benefits and (2) submit a copy of your insurance card showing valid enrollment dates. Curtis will compare the information from your private plan to that of the plan it offers to determine if your private plan coverage is adequate while you are attending Curtis.

If Curtis determines that your private plan provides adequate protection, both you and your parents will be required to sign release forms stating that you (and/or your parents) will be responsible for all uninsured or underinsured medical expenses and that you will not seek reimbursement from Curtis for any medical expenses you might incur.

If Curtis determines that your private plan does not provide adequate protection, you will be advised to enroll in the plan offered through Curtis. If you still choose to be exempt from the Curtis plan, you and your parents will be required to sign the waivers for uninsured medical expenses indicated above.
