Order Transcripts

Curtis supplies transcripts for alumni at a cost of $5 per transcript. Transcripts are mailed within one week of payment, made by check or credit card.

Curtis adheres to the Family Educational Rights and Privacy Act (FERPA). For more information, read the Curtis privacy policy.

Requests (by mail, fax, or online form should include the following information:

• First and last names
• Major
• Class year
• Address
• City, state, zip, country
• Phone
• Fax
• E-mail
• Transcript copy for alumnus/alumna: yes or no

Send transcript to:

• Addressee
• Organization name
• Address
• City, state, zip, country
• Number of transcripts requested
• Total ($5 per transcript)
• Payment information, including credit card expiration date
